Background:

  • Cetuximab is an epidermal growth factor (EGF) receptor antagonist used to treat metastatic colon cancer.
  • EGF receptor antagonists are known to cause acneiform cutaneous side effects due to interference with pilosebaceous follicle homeostasis.

Observation:

  • A 71-year-old man developed an acute acneiform rash on his face and upper trunk two weeks after initiating cetuximab treatment.
  • The rash resolved upon discontinuation of cetuximab and application of topical corticosteroids.
  • Re-exposure to cetuximab led to a relapse of the rash, which was clinically and histologically identified as transient acantholytic dermatosis.

Findings:

  • The patient's skin eruption evolved from an acneiform rash to transient acantholytic dermatosis upon re-exposure to cetuximab.
  • This case suggests a potential pathogenesis related to the acrosyringium, distinct from the typical pilosebaceous unit involvement.

Implications:

  • Transient acantholytic dermatosis may be a cutaneous side effect of EGF receptor antagonists like cetuximab.
  • Management with topical corticosteroids and emollients may allow for continued cetuximab therapy despite skin reactions.
  • Further research into acrosyringium-related pathogenesis of drug-induced dermatoses is warranted.
