[Anesthetic management for a patient with postural hypoxemia]

Summary
Postural hypoxemia, rare in healthy patients, can complicate anesthesia. Maintaining alveolar recruitment and thorax-lung compliance with PEEP and Fowler position improved respiratory mechanics in a surgical patient.

- Respiratory mechanics were monitored using an anesthetic gas analyzer and spirometry device.
Findings:
- Expiratory tidal volume and thorax-lung compliance decreased with elevated end-tidal carbon dioxide (PETCO2) in the supine position and during liver retraction.
- Positive end-expiratory pressure (PEEP) helped maintain alveolar recruitment.
- The Fowler position improved thorax-lung compliance.

- These maneuvers positively influenced the patient's respiratory mechanics during surgery.
- Strategies to manage postural hypoxemia are crucial for anesthetic safety in susceptible patients.
