Cutaneous polyarteritis nodosa and ulcerative colitis.

Summary

Necrotizing vasculitis rarely occurs with ulcerative colitis. This case details a patient with cutaneous polyarteritis nodosa 19 years after ulcerative colitis diagnosis and colectomy.

Background:

Ulcerative colitis (UC) is an inflammatory bowel disease.
Vasculitis, inflammation of blood vessels, is a rare complication of UC.

Observation:

A 48-year-old male with a history of UC presented with necrotizing vasculitis.
The patient had undergone total colectomy with rectal preservation 5 years prior.
The vasculitis affected small arteries in the skin and muscle.

Findings:

The presentation was consistent with cutaneous polyarteritis nodosa (PAN).
Cutaneous PAN has been previously associated with regional enteritis but not UC.
This represents a novel association between UC and cutaneous PAN.

Implications:

This case expands the known spectrum of vasculitic complications in UC patients.
It highlights the importance of considering vasculitis in UC patients with new-onset skin and muscle symptoms.
Further research may elucidate sharedPathogenic mechanisms between UC and vasculitis.
