Summary

Crusted scabies, a severe mite infestation, occurred in a man with acquired selective IgA deficiency. This deficiency was potentially caused by long-term phenytoin medication for epilepsy.

Area of Science:

  • Dermatology
  • Immunology
  • Pharmacology

Background:

  • Crusted scabies is a severe form of scabies infestation, typically seen in immunocompromised individuals or those with significant debility.
  • Acquired selective IgA deficiency is a rare condition characterized by low levels of immunoglobulin A.

Observation:

  • A 40-year-old male patient presented with crusted scabies.
  • The patient had a history of epilepsy treated with long-term phenytoin medication.

Findings:

  • The patient was diagnosed with acquired selective IgA deficiency.
  • Phenytoin use was suspected as the cause of the IgA deficiency, which likely predisposed the patient to crusted scabies.

Implications:

  • This case highlights a potential link between phenytoin-induced IgA deficiency and the development of crusted scabies.
  • Further research is warranted to explore the immunological mechanisms underlying this association.
  • Clinicians should consider monitoring IgA levels in patients on long-term phenytoin therapy, especially those with unusual skin conditions.
