Area of Science:

  • Hematology
  • Oncology
  • Pathology

Background:

  • Idiopathic myelofibrosis (IMF) is a chronic myeloproliferative neoplasm.
  • Key features include bone marrow fibrosis, enlarged spleen, and anemia.
  • A leukoerythroblastic blood smear with dacryocytes is a typical finding.

Observation:

  • This report details a unique case of IMF.
  • The patient presented with classic IMF symptoms but lacked dacryocytes on peripheral blood smear.

Findings:

  • The absence of dacryocytes in this IMF case challenges typical diagnostic criteria.
  • This presentation underscores the heterogeneity of idiopathic myelofibrosis.

Implications:

  • Broadens the understanding of IMF's clinical and hematological presentation.
