Background/Aims:
Genitourinary complications occur in 4 to 35% of Crohn's disease patients. The aim of this study was to assess the threshold to suspect urologic involvement in Crohn's disease in order to plan the correct surgical management.
Methodology:
Medical records of 258 consecutive patients who have undergone bowel resection for Crohn's disease were reviewed. We evaluated recurrent urinary tract infections, fever, dysuria, pneumaturia, fecaluria, abdominal mass at palpation or lower back pain at percussion, abdominal ultrasound and computerized tomography scan reports. Univariate analysis and multivariate analysis were performed with Fisher exact and log-linear tests respectively.
Results:
Urologic complications were found in 11 patients (4.3%). Fistulizing disease, female gender and inflammatory mass were significantly increased in Crohn's disease patients with urinary tract involvement (p < 0.01). Ultrasound and computerized tomography scan demonstrated good specificity, sensibility, positive and negative predicting values for urologic complications.
Conclusions:
In the presence of abdominal mass in a Crohn's disease patient, the following step should be abdominal ultrasound or computerized tomography scan to rule out involvement of the ureter that should be treated previously to improve the intraoperative picture and patient general status.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by transmural...

Chronic Bowel Disorders: Introduction
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
