Area of Science:

  • Vascular Surgery
  • Gastrointestinal Surgery
  • Emergency Medicine

Background:

  • Acute mesenteric ischemia (AMI) is a critical surgical emergency with high mortality rates (59-100%).
  • Delayed diagnosis and unspecific symptoms contribute to poor outcomes in AMI.
  • Effective management of AMI requires prompt surgical intervention.

Purpose of the Study:

  • To present surgical management experience with acute mesenteric ischemia.
  • To analyze outcomes based on intervention timing and surgical approach.

Main Methods:

  • Retrospective analysis of 61 patients surgically treated for AMI between 1988 and 2004.
  • All patients underwent laparotomy, with 75% operated within 24 hours.
  • Surgical procedures included embolectomy, thrombectomy, and bowel resection.

Related Experiment Videos

Main Results:

  • Superior mesenteric artery embolism (59%) and thrombosis (34%) were the primary causes.
  • Mortality and morbidity rates were high at 75% and 80%, respectively.
  • Early surgical intervention (within 24 hours) and embolectomy/thrombectomy with bowel resection improved survival rates (p=0.019).

Conclusions:

  • Acute mesenteric ischemia is a highly lethal condition requiring urgent recognition and treatment.
  • Reducing the time from symptom onset to treatment is crucial for improving patient outcomes.
  • Timely surgical intervention, including revascularization and resection, is key to managing AMI.
