Summary

Orbital tuberculosis, though rare, can mimic other conditions. Early diagnosis and treatment with antitubercular therapy are crucial, especially in patients on immunosuppressants.

Background:

  • Orbital tuberculosis is an uncommon condition.
  • Orbital pseudotumor is a common differential diagnosis for intraorbital lesions.

Observation:

  • A 17-year-old male presented with an intraorbital lesion initially suspected as orbital pseudotumor.
  • The patient showed initial improvement with oral corticosteroids.
  • The condition progressed to a left orbital mass, discharging sinus, and cervical lymphadenitis.

Findings:

  • Diagnosis of tuberculosis was confirmed.
  • The patient responded well to a four-drug antitubercular regimen.

Implications:

  • Corticosteroid therapy may alter the presentation and course of orbital tuberculosis.
  • A high index of suspicion for tuberculosis is necessary in patients presenting with orbital inflammatory conditions, particularly those on immunosuppressive therapy.
