Summary

Dopamine agonist cabergoline, used for Parkinson's disease, may cause constrictive pericarditis (CP). A patient treated with cabergoline developed CP, with thickened pericardium noted except in the myocardial infarction area, suggesting cabergoline as the likely cause.

Area of Science:

  • Cardiology
  • Neurology
  • Pharmacology

Background:

  • Dopamine agonists, such as pergolide and cabergoline, are utilized in Parkinson's disease treatment.
  • Constrictive pericarditis (CP) is a condition involving the thickening and stiffening of the pericardium.

Observation:

  • A 72-year-old male with Parkinson's disease (paralysis agitans) presented with clinical signs of constrictive pericarditis.
  • The patient had a history of myocardial infarction (MI) one year prior and had been on cabergoline therapy for four years.
  • Pericardiectomy revealed a significantly thickened pericardium (1-1.5 cm) encasing the right side of the heart, but the pericardium was normal in the area of the prior MI.

Findings:

  • The study suggests a potential association between cabergoline treatment and the development of constrictive pericarditis.

Related Experiment Videos

  • The localized absence of pericardial thickening over the myocardial infarction area points towards cabergoline as a probable etiological factor, differentiating it from ischemic causes.
  • This case highlights the potential for dopamine agonists to induce cardiac conditions beyond their intended therapeutic effects.
  • Implications:

    • Clinicians should consider the possibility of drug-induced constrictive pericarditis in patients treated with dopamine agonists like cabergoline.
    • Further research is warranted to elucidate the mechanisms by which cabergoline may contribute to pericardial disease.
    • This finding may influence treatment guidelines and patient monitoring for individuals on long-term dopamine agonist therapy for Parkinson's disease.
