Background:

  • Critically ill patients may exhibit novel EEG patterns.
  • Stimulus-induced rhythmic, periodic or ictal discharges (SIRPIDs) represent a recently identified EEG variant.

Observation:

  • A 50-year-old unresponsive female patient with a history of coronary artery bypass surgery presented with symptoms suggestive of brainstem infarction or embolic event.
  • Initial EEG recordings revealed periodic and rhythmical discharges.
  • These discharges were consistently elicited by various forms of stimulation, consistent with SIRPIDs.

Findings:

  • The patient's EEG demonstrated clear evidence of SIRPIDs.
  • The case highlights the occurrence of SIRPIDs in a patient with a suspected acute neurological event.

Implications:

  • This case contributes to the limited understanding of SIRPIDs in critically ill patients.
  • Further investigation is warranted to elucidate the pathophysiology and establish effective treatment strategies for SIRPIDs.
  • Recognizing SIRPIDs may be crucial for accurate diagnosis and management in intensive care settings.
