Background:

  • Dermatitis herpetiformis Duhring is a chronic, intensely pruritic autoimmune blistering skin disease.
  • It is strongly associated with celiac disease and gluten sensitivity.

Observation:

  • A 54-year-old male presented with a three-year history of atypical dermatitis herpetiformis Duhring.
  • Lesions were exclusively located in the pubic area, groins, inner thighs, and perianal regions.

Findings:

  • Histopathology and direct/indirect immunofluorescence confirmed the diagnosis of dermatitis herpetiformis Duhring.
  • A gluten-free diet and dapsone therapy resulted in significant symptom alleviation.

Implications:

  • This case highlights the importance of considering atypical presentations of dermatitis herpetiformis Duhring.
