Summary

The Injury Severity Score (ISS) has limitations in accurately assessing trauma deaths, as crucial factors like underlying conditions are often missed. Improvements are needed for better trauma care quality and standardization.

Area of Science:

  • Trauma Care
  • Forensic Medicine
  • Public Health

Background:

  • The Injury Severity Score (ISS) is a widely used metric for trauma patient assessment.
  • This study compares ISS scores from Emergency Surgery and Trauma Centers (ES&TC) with autopsy findings.
  • Preventable deaths, defined as ISS less than 14, were specifically evaluated.

Purpose of the Study:

  • To compare clinical ISS at admission with autopsy-determined ISS.
  • To identify limitations of the current ISS in evaluating trauma deaths.
  • To assess the rate of preventable trauma deaths and compare findings internationally.

Main Methods:

  • A retrospective analysis of 160 trauma autopsies was conducted.
  • Data were collected from Istanbul Medical Faculty and the Turkish Council of Forensic Medicine between 2000 and 2002.

Related Experiment Videos

  • Injury Severity Scores from clinical admission and autopsy were compared, with a focus on cases with ISS < 14.
  • Main Results:

    • The majority of trauma victims were male (78.1%) with an average age of 32.9 years.
    • Only 3% of autopsies (ISS < or = 14) were classified as "preventable deaths".
    • Clinical evaluations showed higher ISS scores (16-66 and 75) compared to autopsy findings for many cases.

    Conclusions:

    • The ISS has application difficulties and does not fully reflect all contributing factors to mortality, such as underlying diseases or substance use.
    • Evaluating anatomical injuries without physiological changes presents a challenge for ISS accuracy.
    • The ISS requires updates to improve the assessment of clinical care quality and standardize trauma centers.
