Erectile dysfunction (ED) prevalence varies in patients with coronary artery disease (CAD), influenced by clinical presentation and disease severity. In many cases, ED precedes CAD diagnosis by several years.
Area of Science:
- Cardiology
- Urology
- Vascular Medicine
Background:
- Erectile dysfunction (ED) is a common condition that can be associated with cardiovascular diseases.
- Coronary artery disease (CAD) encompasses acute coronary syndrome (ACS) and chronic coronary syndrome (CCS), with varying degrees of vessel involvement.
Purpose of the Study:
- To investigate the prevalence of ED in patients with CAD.
- To analyze ED prevalence based on clinical presentation (ACS vs. CCS) and extent of coronary artery disease (single-vessel vs. multi-vessel disease).
Main Methods:
- A study involving 285 patients with CAD, divided into age-matched groups based on ACS/CCS and vessel involvement.
- Gensini's score was used to quantify CAD severity, and the International Index of Erectile Function (IIEF) assessed ED.
- A control group of 95 patients with normal coronary arteries was included for comparison.
Main Results:
- ED prevalence was significantly lower in patients with ACS and single-vessel disease (22%) compared to those with chronic coronary syndrome (65%), correlating with lower Gensini's scores.
- Patients with ACS and multi-vessel disease showed higher ED rates (55%) and Gensini's scores, indicating ED severity is linked to CAD extent.
- Independent predictors of ED included age, multi-vessel disease, and chronic coronary syndrome compared to acute coronary syndrome.
Conclusions:
- The prevalence of ED in CAD patients is associated with the clinical presentation and extent of coronary artery disease.
- In a majority of patients with established CAD, ED onset precedes the diagnosis of CAD by an average of 2-3 years.
