Area of Science:

  • Cardiovascular Surgery
  • Thoracic Surgery
  • Vascular Surgery

Background:

  • Extensive or multiple aortic pathologies necessitate total aortic resection.
  • Fuwai Cardiovascular Hospital presents its surgical experience with this complex procedure.

Purpose of the Study:

  • To describe the experience with one-stage total or subtotal aortic replacement.
  • To evaluate the feasibility and outcomes of this extensive aortic surgery.

Main Methods:

  • Thirteen Marfan syndrome patients underwent one-stage aortic replacement (Feb 2004-Oct 2005).
  • Procedures involved circulatory arrest, profound hypothermia, and mid-sternotomy with thoracoabdominal incision.
  • Total aortic replacement (ascending aorta to bifurcation) or subtotal replacement (ascending aorta to abdominal aorta) was performed.

Related Experiment Videos

Main Results:

  • No operative or early postoperative deaths occurred.
  • One patient experienced cerebral infarction; no spinal neurologic deficits were observed.
  • All 13 patients survived the follow-up period (4-24 months) with good functional status.

Conclusions:

  • One-stage total or subtotal aortic replacement is feasible for extensive aortic disease.
  • The procedure offers acceptable surgical risks and satisfactory outcomes.
  • This approach mitigates the risk of remnant aortic aneurysm rupture associated with staged procedures.
