Summary

Laparoscopic total colectomy offers similar oncologic outcomes to open surgery for colorectal cancer, with a shorter need for pain relief but a longer operation time.

Area of Science:

  • Colorectal Surgery
  • Surgical Oncology
  • Minimally Invasive Surgery

Background:

  • No prior studies compared laparoscopic versus open total colectomy for colorectal cancers.
  • This study addresses the lack of comparative data, particularly for synchronous colorectal cancers.

Purpose of the Study:

  • To compare short-term clinical outcomes and oncologic results of laparoscopic and open total colectomy or proctocolectomy for colorectal cancers.

Main Methods:

  • Retrospective comparison of 6 laparoscopic and 12 open total colectomies/proctocolectomies for colorectal cancer between 1997-2005.
  • Data collected prospectively for the open group and retrospectively for the laparoscopic group.

Main Results:

  • Laparoscopic colectomy had a longer operative time (427.5 vs 172.5 min) but required less parenteral analgesia (3 vs 5 days).

Related Experiment Videos

  • No significant differences were observed in recovery (bowel function, diet, ambulation, hospital stay), perioperative morbidity, or mortality.
  • Oncologic outcomes, including lymph node retrieval, recurrence, and survival rates, were similar between both surgical approaches.
  • Conclusions:

    • Laparoscopic total colectomy demonstrates comparable short-term clinical and oncologic outcomes to open surgery for colorectal cancer.
    • The primary advantage of laparoscopy is reduced analgesic requirement, balanced by an increased operative time.
