Background:

  • Cholinergic urticaria (CU) is a physical urticaria triggered by increased core body temperature.
  • Standard treatment with antihistamines may not provide adequate symptom control in severe cases.

Observation:

  • A case study of a 22-year-old male with severe, refractory cholinergic urticaria is presented.
  • The patient experienced significant functional impairment despite high-dose antihistamine therapy.
  • Danazol treatment was initiated due to the inadequacy of conventional treatments.

Findings:

  • Danazol treatment led to significant improvement in urticaria control for the patient.
  • The patient's symptoms, which were refractory to antihistamines, were effectively managed with danazol.
  • This suggests danazol can be effective in severe, treatment-resistant cases of cholinergic urticaria.

Implications:

  • Danazol represents a viable therapeutic option for severe and refractory cholinergic urticaria.
  • Its use should be considered when standard treatments are insufficient.
  • Careful consideration of potential adverse effects is necessary, reserving danazol for the most severe cases.
