Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Microbiology

Background:

  • Uveitis can manifest from various infectious agents.
  • Borrelia hermsii is a known cause of relapsing fever, typically transmitted by ticks.

Observation:

  • A 12-year-old boy presented with symptoms of Borrelia hermsii relapsing fever.
  • Following initial antibiotic treatment, the patient developed anterior and intermediate uveitis with blurred vision and floaters.

Findings:

  • Elevated Borrelia hermsii immunoglobulin M and G levels confirmed the infection.
  • Despite initial treatment with ceftriaxone and cephuroxime, uveitis developed.
  • Subsequent treatment with doxycycline and topical corticosteroids resulted in improved visual acuity and resolution of uveitis.

Related Experiment Videos

Implications:

  • This case suggests Borrelia hermsii should be considered in the differential diagnosis of infectious uveitis.
  • Early recognition and appropriate antimicrobial therapy are crucial for managing Borrelia-associated ocular complications.
