Asymptomatic patients with congenitally corrected transposition of the great arteries (ccTGA) show early signs of right ventricular (RV) dysfunction. Echocardiography reveals subtle RV abnormalities, indicating the need for closer monitoring in these patients.

Background:

  • Right ventricular (RV) failure is a common complication in congenital heart defects where the RV supports systemic circulation.
  • RV failure often develops gradually, and asymptomatic patients may harbor unrecognized RV dysfunction.

Purpose of the Study:

  • To investigate subclinical RV dysfunction in asymptomatic patients with systemic RV due to congenitally corrected transposition of the great arteries (ccTGA).
  • To utilize advanced echocardiographic techniques to detect early signs of RV impairment.

Main Methods:

  • Conventional and Doppler tissue/strain echocardiography were performed on 13 asymptomatic ccTGA patients and 10 age-matched controls.
  • RV index of myocardial performance (RIMP) was measured.

  • Longitudinal tissue velocities, strain rate, and strain of the basal RV free wall were assessed.

    • Patients with ccTGA had a significantly higher RIMP compared to controls (0.66 +/- 0.25 vs 0.28 +/- 0.12, P < .001).
    • RV tissue displacement, peak systolic strain rate, and peak systolic strain were significantly lower in ccTGA patients.
    • These findings indicate reduced RV contractility and deformation in asymptomatic ccTGA individuals.

    Conclusions:

    • Asymptomatic patients with ccTGA exhibit quantifiable RV dysfunction detectable by echocardiography.
    • Early identification of RV dysfunction is crucial for managing patients with systemic RV due to ccTGA.
