Summary

This report details the first South African case of fibroblastic rheumatism, a rare condition causing joint and skin issues. Early diagnosis and treatment with methotrexate and prednisolone helped manage symptoms and prevent further joint damage.

Background:

  • Fibroblastic rheumatism is a rare dermatoarthropathy.

  • The patient exhibited sudden-onset erosive polyarthritis, cutaneous nodules, and sclerodactyly.
  • Rapid functional loss occurred within 3-4 months.
  • Imaging revealed erosions and soft tissue/synovial enhancement.

Findings:

  • Histopathology confirmed fibroblastic proliferation, thickened collagen, and dermal fibrosis.
  • Diagnosis of fibroblastic rheumatism was established.
  • Treatment with methotrexate and prednisolone resolved synovitis/arthritis.
