An unusual beta-cell adenoma.

J Dewilde, J Baillargeon, P Daloze

    Summary

Diagnosing hypoglycemia can be challenging, as shown in a case of insulinoma. Sporadic, inappropriate insulin levels were key to identifying a pancreatic beta-cell adenoma causing low blood sugar.

    Area of Science:

    • Endocrinology
    • Oncology
    • Gastroenterology

    Background:

    • Hypoglycemia, or low blood glucose, presents diagnostic challenges.
    • Insulinoma, a rare pancreatic tumor, is a common cause of endogenous hyperinsulinemic hypoglycemia.

    Observation:

    • A 51-year-old male presented with symptoms suggestive of hypoglycemia.
    • Extensive laboratory and procedural investigations were conducted.
    • Sporadic findings of inappropriately elevated plasma immunoreactive insulin relative to blood glucose levels were noted.

    Findings:

    • A small beta-cell adenoma located in the pancreatic head was identified as the source of hypoglycemia.
    • The diagnostic difficulty stemmed from the inconsistent nature of the biochemical markers.

    Implications:

    • This case highlights the importance of persistent and comprehensive diagnostic evaluation for elusive insulinoma.
    • Understanding the variable presentation of insulinoma is crucial for timely diagnosis and effective patient management.
    • Further research into improved diagnostic markers for insulinoma may be warranted.
