Summary

Chronic widespread pain (CWP) frequently co-occurs with other conditions, and familial factors significantly influence these associations. Genetic and environmental influences explain much of the comorbidity observed in the general population.

Background:

  • Chronic widespread pain (CWP) is a prevalent symptom of fibromyalgia, often co-occurring with conditions like chronic fatigue, joint pain, headache, IBS, and psychiatric disorders.
  • Few studies have investigated CWP comorbidities in the general population.
  • The role of familial factors (genetic and environmental) in CWP comorbidity etiology is largely unknown.

Purpose of the Study:

  • To examine the comorbidities of CWP in a large general population sample.
  • To assess the influence of familial factors on the co-occurrence of CWP and its associated conditions.

Main Methods:

  • Utilized data from 44,897 individuals in the Swedish Twin Registry (age >/=42 years).
  • Screened for CWP using American College of Rheumatology criteria (without clinical evaluation).

  • Employed case-control and co-twin control designs to analyze associations and familial confounding.
  • Main Results:

    • Significant co-occurrences found between CWP and chronic fatigue (OR 23.53), joint pain (OR 7.41), depressive symptoms (OR 5.26), and irritable bowel syndrome (OR 5.17).
    • Co-twin control analyses revealed that familial factors mediated associations with psychiatric disorders and most other comorbidities.
    • Headache associations with CWP showed no change in odds ratios after accounting for familial factors.

    Conclusions:

    • Unmeasured genetic and family environmental factors significantly mediate the associations between CWP and most comorbidities in the general population.
    • The degree of mediation by familial factors appears to be specific to each comorbidity.
    • These findings highlight the complex interplay of genetic and environmental influences in the manifestation of CWP and its related conditions.
