Vasa vasorum neovascularization increases blood flow and exchange surface in the proximal LAD during early atherosclerosis. This may explain why complex atherosclerotic lesions predominantly develop in these areas.

Conclusions:
- VV neovascularization in early atherosclerosis accentuates proximal flow capacity and endothelial exchange.
- This proximally increased vascularization may promote the development of vulnerable atherosclerotic plaques in the proximal LAD.
