Confidential reporting of coronary artery bypass graft (CABG) surgery outcomes in Ontario led to a significant 29% reduction in mortality. Public reporting did not yield further improvements, suggesting confidential disclosure is key for quality enhancement.
Area of Science:
- Health Services Research
- Quality Improvement Science
- Cardiac Surgery Outcomes
Background:
- Ontario has a history of generating institution-level performance report cards for coronary artery bypass graft (CABG) surgery over 11 years.
- The study examines changes in patient characteristics and outcomes during the transition from no reporting to confidential and public performance report cards.
Purpose of the Study:
- To evaluate the impact of different performance reporting strategies (none, confidential, public) on CABG surgery outcomes in Ontario.
- To assess whether reporting led to improvements in patient mortality and other health outcomes.
Main Methods:
- Utilized clinical and administrative data for 67,693 isolated CABG patients from 1991 to 2002.
- Assessed crude, expected, and risk-adjusted 30-day mortality rates, comparing trends across reporting eras.
- Used bootstrapping for confidence intervals and compared 30-day readmission rates as a control outcome.
Main Results:
- Risk-adjusted 30-day mortality decreased by 29% from no reporting to confidential reporting (1991-1998).
- No significant mortality reduction was observed with subsequent public reporting (1999-2001).
- In-hospital mortality declined faster in Ontario than in the rest of Canada during confidential reporting.
Conclusions:
- Confidential, institutional-level disclosure of CABG outcomes significantly improved mortality rates in Ontario.
- Public reporting of outcomes did not demonstrate a further impact on performance.
- Confidential disclosure appears sufficient to drive quality improvement in public healthcare systems.
