Sertindole: efficacy and safety in schizophrenia

Sertindole is an effective antipsychotic with a low risk of extrapyramidal side effects and sedation. While it may prolong QTc interval, it offers an alternative for patients unresponsive to other treatments.
Area of Science:
- Pharmacology
- Neuroscience
- Psychiatry
Background:
- Sertindole is an atypical antipsychotic targeting dopamine D2, serotonin 5-HT2A/5-HT2C, and alpha1-adrenoreceptors.
- Preclinical data indicate preferential action on limbic and cortical dopaminergic neurons.
- Clinical trials confirm efficacy at low dopamine D2 occupancy levels.
Purpose of the Study:
- To evaluate the efficacy and safety profile of sertindole as an antipsychotic medication.
- To compare sertindole's effects with other antipsychotics, particularly risperidone.
- To identify patient populations that may benefit from sertindole treatment.
Main Methods:
- Review of preclinical studies and clinical trial data.
- Analysis of pharmacodynamic properties, including receptor affinity and D2 occupancy.
- Examination of adverse event profiles and comparison with placebo and other antipsychotics.
- Assessment of efficacy in positive and negative symptoms of schizophrenia.
Main Results:
- Sertindole demonstrates efficacy at low dopamine D2 occupancy, with a long half-life and stable plasma levels.
- Low incidence of extrapyramidal side effects and tardive dyskinesia observed.
- Sertindole lacks sedative effects, unlike many other antipsychotics.
- QTc prolongation is a noted side effect, but large analyses do not link sertindole to increased all-cause mortality.
- Equivalent efficacy to risperidone for positive symptoms; mixed results for negative symptoms.
- Adverse events like weight gain, rhinitis, and decreased ejaculation volume were more common than placebo.
Conclusions:
- Sertindole is an effective antipsychotic with a favorable side-effect profile regarding extrapyramidal symptoms and sedation.
- Its QTc prolonging effect necessitates caution, precluding its use as a first-line treatment for first-episode schizophrenia.
- Sertindole presents a valuable alternative for patients with treatment-resistant schizophrenia or those who do not respond well to initial antipsychotic choices.
