Summary

The BCT analyzer effectively monitors aspirin and clopidogrel response using platelet aggregation, unlike the PFA-100, which is limited to aspirin. This study assesses antiplatelet drug efficacy in patients with arterial occlusive disease.

Area of Science:

  • Cardiology
  • Hematology
  • Clinical Chemistry

Background:

  • Monitoring antiplatelet drug efficacy is crucial for patients with arterial occlusive disease.
  • Laboratory non-response to aspirin or clopidogrel indicates inadequate platelet function inhibition.
  • Widely available methods like the Platelet Function Analyzer (PFA-100) and automated coagulation analyzer BCT could potentially monitor drug response.

Purpose of the Study:

  • To evaluate the BCT and PFA-100 analyzers for monitoring aspirin and clopidogrel response.
  • To investigate the use of platelet-enriched plasma (PRP) in aggregation studies.
  • To assess clopidogrel's effect on platelet function independently of aspirin.

Main Methods:

  • Investigated 125 patients with arterial occlusive disease on aspirin or clopidogrel monotherapy.

Related Experiment Videos

  • Utilized the BCT and PFA-100 for platelet function analysis.
  • Performed arachidonic acid-induced and ADP-induced aggregation tests.
  • Main Results:

    • The BCT detected aspirin response in 91% of patients, while PFA-100 detected it in 85%.
    • Non-response to aspirin was identified in 7% of patients by both methods.
    • The BCT showed clopidogrel response in 58% of patients; PFA-100 could not detect clopidogrel's antiplatelet effect.

    Conclusions:

    • The BCT analyzer, using native PRP, accurately quantifies individual inhibitory effects of both aspirin and clopidogrel.
    • The PFA-100 is suitable for monitoring aspirin's effect but not clopidogrel's.
    • BCT offers a more comprehensive tool for monitoring antiplatelet therapy response.
