Area of Science:

  • Plastic Surgery
  • Otolaryngology
  • Facial Aesthetic Surgery

Background:

  • Traditional open rhinoplasty involves a transcolumellar incision, invariably leaving a visible scar.
  • This scar is particularly problematic and noticeable in non-Caucasian patients.
  • Minimizing visible scarring is a significant goal in reconstructive and aesthetic nasal surgery.

Purpose of the Study:

  • To present a modified open rhinoplasty technique that eliminates the transcolumellar incision.
  • To achieve adequate exposure of the nasal framework without compromising surgical outcomes.
  • To provide an alternative for patients concerned about columellar scarring.

Main Methods:

  • Bilateral paramarginal incisions were extended to the nasal floors, circumventing the columella.

  • Lower lateral cartilages were delivered through a nostril for nasal tip manipulation.
  • This technique allows for visualization similar to traditional open rhinoplasty.
  • Main Results:

    • The modified technique provided wide exposure of the nasal framework.
    • Satisfactory aesthetic and functional results were achieved in 23 patients.
    • No transcolumellar scar was present in any of the treated patients.

    Conclusions:

    • Open rhinoplasty without a transcolumellar incision is a viable alternative to the traditional technique.
    • This scarless approach is suitable for selected patients, particularly those concerned about visible scarring.
    • The technique offers comparable surgical exposure and satisfactory outcomes.
