Summary

Patella maltracking in total knee arthroplasty is often avoidable. A new posterior-stabilized knee implant and protocol resulted in a low incidence of patellofemoral complications and lateral retinacular release.

  • Data collection focused on the need for lateral retinacular release and subsequent patellofemoral complications.

    • Lateral retinacular release was performed in 6.2% of cases, primarily in patients with significant preoperative valgus deformity.
    • No reoperations for patellofemoral joint issues were required at a mean follow-up of 3.7 years.
    • Two patients developed asymptomatic osteonecrosis of the patella, and one patella fracture occurred, managed non-operatively.

    Conclusions:

    • Patella maltracking appears to be a largely preventable complication in TKA.

    • This approach may contribute to improved long-term outcomes in total knee replacement surgery.
