  • To evaluate PDS for improving biopsy core management and cancer staging.

  • 579 patients with PSA > 3.5 ng/ml underwent initial sextant biopsies.
  • PDS was performed prior to biopsy indication.
  • Additional Doppler-guided biopsies were performed for persistent elevated PSA.

Main Results:

  • PDS detected vascular anomalies corresponding to 260 cancers; 39 were false negatives.
  • An abnormal Doppler signal predicted an 83% risk of positive additional biopsies.
  • Vascular patterns correlated with Gleason score and extraprostatic extension.
