Summary

Antepartum transabdominal amnioinfusion did not improve the success rate of external cephalic version in women with breech presentation and low amniotic fluid. This procedure was ineffective in facilitating the version in our study group.

Background:

  • External cephalic version (ECV) is a procedure to turn a breech fetus to a cephalic presentation.
  • Low amniotic fluid index (AFI) can be a barrier to successful ECV.
  • Antepartum amnioinfusion has been explored as a method to increase AFI and potentially facilitate ECV.

Purpose of the Study:

  • To assess the efficacy of transabdominal amnioinfusion in improving the success rate of ECV after an initial attempt failed.
  • To determine if increasing amniotic fluid volume can aid in fetal version.

Main Methods:

  • A study involving women at term with a breech fetus and AFI < 15 cm after a failed ECV.
  • Participants received tocolysis with indomethacin, followed by transabdominal amnioinfusion to increase AFI to at least 15 cm (max 1 L).
  • ECV was attempted again immediately after the amnioinfusion.

Main Results:

  • Seven women participated, with gestational ages between 36 and 38 weeks.
  • The AFI ranged from 4 to 13 cm; a median of 1000 mL of Lactated Ringers solution was infused.
  • None of the repeat ECV attempts were successful after amnioinfusion.

Conclusions:

  • Antepartum transabdominal amnioinfusion did not facilitate external cephalic version in this cohort.
  • The procedure, as performed in this study, did not improve ECV success rates in cases of initial failure and low AFI.
