Summary

Postdischarge costs for primary arthroplasty are substantial. Discharging patients directly home, rather than to a comprehensive rehabilitation unit, significantly reduces costs without compromising short-term outcomes.

Area of Science:

  • Orthopedic Surgery
  • Health Economics

Background:

  • Postdischarge costs following primary arthroplasty are not well-documented.
  • Understanding these expenditures is crucial for healthcare cost management.

Purpose of the Study:

  • To identify and quantify postdischarge costs for primary arthroplasty patients.
  • To estimate the annual national expenditure on postsurgical rehabilitation after arthroplasty.

Main Methods:

  • A cohort of 136 primary arthroplasty patients was analyzed.
  • Costs for comprehensive rehabilitation unit (CRU) and home care (HC) were collected.
  • National discharge cost estimates were modeled using 2003 National Hospital Discharge Survey data.

Main Results:

  • Total postdischarge costs were significantly lower for patients discharged directly home ($2405) compared to those discharged to a CRU ($13435).

Related Experiment Videos

  • Both patient groups demonstrated similar improvements in quality of life.
  • An estimated $3.2 billion is spent annually on postsurgical rehabilitation in the US.
  • Conclusions:

    • Discharge destination significantly impacts postdischarge costs in primary arthroplasty.
    • Direct home discharge is a more cost-effective strategy than CRU admission for comparable short-term outcomes.
