Background:

  • Primary adrenal lymphoma is an exceptionally rare malignancy.
  • Adrenal masses can be challenging to diagnose, especially when presenting with non-specific symptoms.

Observation:

  • Two patients presented with adrenal insufficiency or fever of unknown origin.
  • Imaging revealed bilateral, bulky adrenal masses as the primary finding.
  • Whole-body FDG-PET-CT confirmed adrenal masses as the sole manifestation.

Findings:

  • Pathological diagnosis for both patients was non-Hodgkin lymphoma.
  • FDG-PET-CT is valuable in identifying the extent of primary adrenal lymphoma.

Implications:

  • Primary adrenal lymphoma should be considered in the differential diagnosis of bilateral adrenal masses.
