The kidneys receive blood directly from the heart via the renal arteries. Background:

  • Thyroid dysfunction is known in end-stage renal disease (ESRD).
  • Data on thyroid function and morphology post-renal transplantation (RT) are inconsistent.
  • Successful RT may impact the hypothalamic-pituitary-thyroid (HPT) axis.

Purpose of the Study:

  • To determine the prevalence of thyroid function and morphology disorders after successful RT.
  • To identify risk factors associated with these post-transplant thyroid abnormalities.
  • To guide appropriate endocrinological follow-up strategies.

Main Methods:

  • Cross-sectional, case-control study design.
  • Fifty RT patients and 100 healthy controls were enrolled.
  • Assessed serum thyroid hormones, TSH, thyroglobulin, TBG, urinary iodine, and thyroid ultrasound.

Main Results:

  • 52% of RT patients had "low T3 syndrome"; 26% showed suppressed HPT axis function.
  • 24% of patients had thyroid nodules, linked to longer dialysis duration.
  • Chronic thyroiditis patterns were more common than biochemical evidence.

Conclusions:

  • High prevalence of abnormal thyroid morphology post-RT, partly due to prior uremia.
  • Thyroid function abnormalities reflect post-transplant conditions.
  • Endocrinological monitoring is crucial to differentiate treatable from manageable thyroid issues.
