  • Patients received kidneys from blood group incompatible donors (A to AB, O to A).

Findings:

  • Preconditioning included tacrolimus, mycophenolate mofetil, and methylprednisolone.
  • Double filtration plasmapheresis (DFPP) effectively reduced anti-donor ABO antibodies.
  • Successful transplantation achieved with low serum creatinine levels post-procedure (1.0 and 1.4 mg/dL).

Implications:

  • ABO-incompatible renal transplantation is feasible with modern immunosuppression, DFPP, and splenectomy.
  • This approach expands options for patients with limited donor availability.
  • Successful outcomes demonstrate the potential to overcome blood group barriers in live donor kidney transplantation.
