Summary

Extrapulmonary small cell carcinoma (ESCC) occurs in various sites, most commonly the esophagus. Limited disease-ESCC shows a significantly better prognosis than extensive disease-ESCC, with multimodality therapy being key.

Area of Science:

  • Oncology
  • Pathology
  • Clinical Medicine

Context:

  • Extrapulmonary small cell carcinoma (ESCC) is a distinct clinicopathologic entity.
  • ESCC predominantly affects the lung but can arise in various extrathoracic sites.
  • Understanding ESCC's characteristics is crucial for effective management.

Purpose:

  • To investigate the clinical characteristics of ESCC.
  • To analyze therapeutic approaches for ESCC.
  • To determine the prognostic factors influencing ESCC patient outcomes.

Summary:

  • A retrospective analysis of 53 ESCC patients (1985-2005) revealed the esophagus as the most common primary site (62.3%).
  • Limited disease (LD-ESCC) patients (75.5%) received diverse treatments including surgery, radiotherapy, and chemotherapy, while extensive disease (ED-ESCC) patients primarily received platinum-based chemotherapy.

  • Median survival time was 20 months overall, with LD-ESCC patients demonstrating significantly better survival (26 months) compared to ED-ESCC patients (15 months).
  • Impact:

    • ESCC presents diverse clinical features and primary sites, with the esophagus being most frequent.
    • Multimodality therapy is increasingly utilized for LD-ESCC, while combination chemotherapy is standard for ED-ESCC.
    • Prognosis is significantly better for LD-ESCC than ED-ESCC, highlighting the importance of disease extent in treatment planning and outcomes.
