Summary

Small bowel transplantation (SBTx) survival now exceeds 90%, matching other organ transplants. This procedure improves quality of life and offers a cost-effective alternative to total parenteral nutrition (TPN) for intestinal failure.

Area of Science:

  • Transplantation immunology
  • Gastroenterology
  • Surgical innovation

Background:

  • Small bowel transplantation (SBTx) historically had poor outcomes compared to other solid organ transplants.
  • Challenges included significant immunologic burden, histocompatibility antigen expression, and graft bacterial contamination.

Purpose of the Study:

  • To evaluate recent advancements in SBTx outcomes.
  • To compare SBTx with home total parenteral nutrition (TPN) for intestinal failure.
  • To assess the potential for expanding SBTx indications.

Main Methods:

  • Implementation of novel immune-induction therapies, including recipient preconditioning and lymphocyte reduction.
  • Utilization of single-agent immunosuppression with tacrolimus.
  • Analysis of graft and host survival rates in leading US centers.

Main Results:

  • 1-year graft and host survival rates now exceed 90% in active US centers.
  • SBTx demonstrates improved quality of life and restores digestive/absorptive function compared to TPN.
  • The procedure is cost-effective with survival beyond 1-3 years.

Conclusions:

  • SBTx outcomes have significantly improved, paralleling liver and kidney transplantation.
  • SBTx offers nutritional autonomy and enhanced quality of life for patients with intestinal failure.
  • Indications for SBTx should expand from salvage to preemptive therapy.
