Summary

Background:

  • Native radiocephalic (Brescia-Cimino) fistulas are typically created by connecting the cephalic vein and radial artery.
  • Catheter interventions for native fistula failure have traditionally used transcephalic or transbrachial access routes.

Purpose of the Study:

  • To prospectively evaluate the safety and feasibility of transradial intervention for native fistula failure.
  • To assess technical and clinical success, complication rates, and patency in patients undergoing transradial intervention for failing radiocephalic fistulas.

Main Methods:

  • Prospective evaluation of 11 consecutive patients with native radiocephalic fistula failure.
  • Interventions included balloon angioplasty, catheter-directed thrombo-aspiration, mechanical thrombectomy, and Cutting Balloon angioplasty.
  • Lesions were characterized as stenotic or occlusive with thrombus.

Main Results:

  • Technical and clinical success was achieved in all 11 patients.
  • No major complications such as vessel rupture, perforation, distal embolization, or pulmonary embolism were observed.
  • Primary patency rates were 82% at 3 months and 64% at 6 months.

Conclusions:

  • Transradial intervention for native radiocephalic fistula failure is a safe and feasible option in selected patients.
  • The procedure demonstrated good short-term patency and a low complication rate.
  • Further validation through larger studies is warranted to confirm these findings.
