Capsule endoscopy (CE) is valuable for diagnosing obscure gastrointestinal bleeding (OGIB). It shows a higher diagnostic yield in overt OGIB cases, aiding treatment planning.
Area of Science:
- Gastroenterology
- Medical Imaging
- Diagnostic Endoscopy
Background:
- Obscure gastrointestinal bleeding (OGIB) is a frequent reason for capsule endoscopy (CE).
- Existing reports show considerable variability in the diagnostic yield of CE for OGIB.
- This study aimed to assess CE's diagnostic yield and clinical impact in OGIB patients.
Purpose of the Study:
- To evaluate the diagnostic yield of capsule endoscopy (CE) in patients presenting with obscure gastrointestinal bleeding (OGIB).
- To determine the clinical impact of CE findings on subsequent patient management.
- To compare the diagnostic efficacy of CE in overt versus occult OGIB.
Main Methods:
- A retrospective review of medical records was conducted.
- Data from 154 patients with OGIB who underwent capsule endoscopy (CE) between June 2002 and October 2005 were analyzed.
- Patient demographics, CE findings, and subsequent management were recorded.
Main Results:
- The overall positive diagnostic yield for CE in OGIB was 52%.
- CE demonstrated a significantly higher diagnostic yield in overt OGIB (77%) compared to occult OGIB (27%).
- Common findings included NSAID-induced lesions (15%), angiodysplasias (14%), and aphthous ulcers (12%). CE influenced management in 79% of overt OGIB and 26% of occult OGIB cases.
Conclusions:
- Capsule endoscopy (CE) is an effective diagnostic tool for patients with obscure gastrointestinal bleeding (OGIB).
- CE is particularly beneficial for patients with overt OGIB, offering a higher diagnostic yield and guiding management decisions.
- The findings support the utility of CE in the diagnostic workup of OGIB.
