Background:

Traumatic amputation of all four limbs is exceptionally rare.
Extensive soft tissue loss precluded immediate, standard hand replantation in this case.

Observation:

A 27-year-old male patient presented with complete amputation of all four limbs.
The right hand was replanted to the left forearm due to extensive tissue damage in both upper limbs.

Findings:

Successful osteosynthesis, vascular, nerve, and tendon repair were performed.
Within one year, protective sensation returned to the replanted hand, though intrinsic muscle paralysis persisted.
Secondary procedures included thumb opposition transfer and capsulorrhaphy to establish a key-grip function.

This case demonstrates the feasibility of cross-limb replantation in severe traumatic injuries.
The staged reconstructive approach can restore significant hand function and improve patient quality of life.
