Women treated for non-ST-segment elevation acute coronary syndromes (NSTE ACS) experience higher bleeding rates, often due to excessive glycoprotein (GP) IIb/IIIa inhibitor dosing. Optimizing GP IIb/IIIa inhibitor dosage can significantly reduce this sex-related bleeding risk in NSTE ACS patients.
Area of Science:
- Cardiology
- Pharmacology
- Clinical Medicine
Background:
- Glycoprotein (GP) IIb/IIIa inhibitors are crucial for non-ST-segment elevation acute coronary syndromes (NSTE ACS).
- Concerns exist regarding the safe use of GP IIb/IIIa inhibitors in women.
- The impact of dosing on sex-specific bleeding differences is not well understood.
Purpose of the Study:
- To investigate the relationship between patient sex, GP IIb/IIIa inhibitor dosing, and bleeding risk in NSTE ACS.
- To determine the contribution of excessive dosing to sex-based bleeding disparities.
Main Methods:
- Analysis of 32,601 NSTE ACS patients from the CRUSADE registry.
- Definition of excessive GP IIb/IIIa inhibitor dose based on creatinine clearance.
- Assessment of major bleeding events and adjusted odds ratios (ORs) by sex.
Main Results:
- Women had higher major bleeding rates than men, both with and without GP IIb/IIIa inhibitor treatment.
- Treated women were significantly more likely to receive excessive GP IIb/IIIa inhibitor doses (46.4% vs. 17.2%).
- Excessive dosing increased bleeding risk in both sexes, but the attributable risk was substantially higher in women (25.0% vs. 4.4%).
Conclusions:
- Women experience greater bleeding than men with NSTE ACS, irrespective of GP IIb/IIIa inhibitor use.
- Excessive dosing in women accounts for a significant portion of the sex-related bleeding difference.
- Appropriate GP IIb/IIIa inhibitor dosing is essential for improving patient care, particularly for women with NSTE ACS.
