Summary

Current human immunodeficiency virus (HIV) treatment involves highly active antiretroviral treatment (HAART) with multiple drugs. Optimal HAART regimens can lead to durable HIV suppression and a normal lifespan for patients.

Area of Science:

  • Infectious Diseases
  • Virology
  • Pharmacology

Background:

  • The landscape of human immunodeficiency virus (HIV) treatment has evolved significantly since 1987.
  • Over 20 antiretroviral agents are now available for managing HIV/AIDS.

Purpose of the Study:

  • To review current insights and standard of care for HIV treatment.
  • To highlight the goals and strategies in managing HIV infection.

Main Methods:

  • Review of current antiretroviral agents and treatment guidelines.
  • Discussion of highly active antiretroviral treatment (HAART) regimens.
  • Emphasis on patient-specific tailoring of treatment based on safety and tolerance.

Main Results:

  • Highly active antiretroviral treatment (HAART), a combination of antiretrovirals, is the standard of care.
  • First-line HAART typically includes 2 nucleoside reverse transcriptase inhibitors plus a nonnucleoside or protease inhibitor.
  • Side effect profiles are increasingly important in selecting initial regimens over efficacy differences.
  • Simultaneous initiation of all medications is crucial to prevent viral resistance.
  • Salvage therapy with complex regimens can reestablish viral control even in treatment-resistant cases.

Conclusions:

  • Durable HIV suppression is achievable with optimal treatment regimens.
  • A normal lifespan is a realistic outcome for individuals receiving modern HIV treatment.
