Computed tomography (CT)-guided percutaneous abscess drainage (PAD) offers a successful treatment for gynecologic abscesses, including tubo-ovarian abscesses and pyometra, when antibiotics fail. This minimally invasive approach provides diagnostic, temporizing, or alternative surgical options.

Findings:
- CT-guided transabdominal percutaneous abscess drainage (PAD) and irrigation successfully treated the TOA in the first patient, with no recurrence at one year.
- CT-guided transabdominal PAD and irrigation were used for the pyometra in the second patient due to cervical os obstruction, leading to clinical improvement.
- The procedure served as a diagnostic aspiration, temporizing measure, and alternative to surgery for both gynecologic abscess cases.
