Background:

  • Hormone-resistant prostate cancer (HRPC) presents a significant clinical challenge.
  • Effective treatment options for HRPC, particularly those improving subjective symptoms and survival, are limited.

Purpose of the Study:

  • To evaluate the efficacy and toxicity of flutamide in patients with hormone-resistant prostate cancer.
  • To assess the correlation between prostate-specific antigen (PSA) decline and survival outcomes.

Main Methods:

  • A cohort of 25 fully evaluable patients with HRPC received flutamide (250 mg orally 3 times daily).
  • Subjective response, PSA levels, and toxicity were monitored.
  • Correlation between PSA decrease and survival was analyzed.

Main Results:

  • Flutamide yielded a subjective response in 5 of 25 patients (20%).
  • A ≥40% decrease in PSA was observed in 7 of 24 patients (29%), correlating with improved survival.
  • Gastrointestinal toxicity led to discontinuation in 5 patients.

Conclusions:

  • Flutamide may offer a role in managing HRPC for symptom relief and potential survival benefit.
  • Further phase 3 trials are essential to validate flutamide's effectiveness, especially against less expensive alternatives.
