Summary

Psychological disturbances in Graves' disease improve with treatment, with significant changes in anxiety and irritability upon achieving euthyroidism. Other psychological factors normalize within months, irrespective of treatment type.

Area of Science:

  • Endocrinology
  • Psychiatry
  • Immunology

Background:

  • Graves' disease is associated with psychological disturbances, but the timeline for recovery during treatment is unclear.
  • Previous research shows conflicting results on the correlation between thyroid hormone levels and psychological/neurophysiological outcomes in Graves' disease.
  • Psychological factors have been hypothesized as potential causes of Graves' disease.

Purpose of the Study:

  • To investigate the time course of psychological disturbances during antithyroid drug treatment for Graves' disease.
  • To examine the relationship between thyroid hormone levels and psychological test results.
  • To explore potential links between psychological symptoms and immune system alterations in Graves' disease.

Main Methods:

Related Experiment Videos

  • Sequential psychological testing was performed on patients with Graves' disease during treatment.
  • Patients underwent assessment for anxiety, irritability, depression, activity, exhaustion, well-being, extraversion, introversion, and concentration.
  • Thyroid hormone levels and T-lymphocyte subsets were analyzed in relation to psychological findings.
  • Main Results:

    • A significant reduction in anxiety and irritability was observed upon achieving euthyroidism.
    • Self-reported measures of depression, activity, exhaustion, well-being, extraversion, introversion, and concentration improved 1-2 months after euthyroidism.
    • Treatment modality (antithyroid drugs vs. surgery) did not affect the normalization of psychological parameters.
    • Minimal correlation was found between thyroid hormone levels and psychological test results.
    • Patients with high depression and anxiety scores showed abnormal helper/suppressor T-lymphocyte ratios.

    Conclusions:

    • Psychological disturbances in Graves' disease, including anxiety and irritability, significantly improve once euthyroidism is achieved.
    • The normalization of psychological parameters occurs within a few months post-euthyroidism and is independent of the treatment method.
    • While thyroid hormone levels show little correlation with psychological status, alterations in T-lymphocyte subsets may be linked to severe depression and anxiety in Graves' disease patients.
