Summary

Growth hormone deficiency (GHD) in adults is a clinical syndrome linked to cardiovascular risks. GH replacement therapy can improve health status and reverse some biological changes in GHD adults.

Area of Science:

  • Endocrinology
  • Metabolic Disorders
  • Cardiovascular Health

Background:

  • Adult growth hormone deficiency (GHD) was historically viewed as a marker of pituitary disease.
  • GHD in adults is now recognized as a clinical syndrome associated with cardiovascular risk factors.
  • These include altered body composition, insulin resistance, adverse lipid profiles, reduced physical performance, and impaired quality of life.

Purpose of the Study:

  • To review the established benefits and ongoing considerations of growth hormone (GH) replacement therapy in adults with GHD.
  • To highlight the current recommendations and remaining questions regarding GH therapy efficacy and safety.

Main Methods:

  • Review of randomized placebo-controlled trials and established clinical guidelines.
  • Analysis of the impact of GH replacement on cardiovascular risk factors and overall health status.
  • Consideration of safety aspects, including long-term effects and monitoring.

Main Results:

  • GH replacement therapy has been shown to reverse some biological changes associated with GHD.
  • Improvements in body composition, lipid profiles, and overall health status have been observed.
  • Recombinant human GH offers a safe and virtually unlimited supply for therapeutic use.

Conclusions:

  • GH replacement therapy is approved in many countries for GHD adults, with a six-month trial recommended.
  • Individual variability in response necessitates monitoring, with IGF-I as a key marker.
  • Further research is needed on partial GHD, elderly populations, and long-term safety, emphasizing the need for surveillance databases.
