Background:

Total lower eyelid defects require complex reconstruction.
Evaluating established and novel reconstructive techniques is crucial for optimal patient outcomes.

Observation:

Ten patients underwent total lower eyelid reconstruction using a paranasal flap for the anterior lamella and various grafts for the posterior lamella.
Follow-up ranged from 3 to 35 months, with patient-reported aesthetic evaluations at 3 months post-surgery.

Findings:

All paranasal flaps and grafts survived without partial or total necrosis.
One patient required a secondary procedure for ectropion, but overall aesthetic results were satisfactory.
The paranasal flap technique proved reliable and safe for total lower eyelid reconstruction.
