Background:

- A 63-year-old male presented with chest pain and shortness of breath.
- A 64-row multidetector computed tomography (MDCT) scan revealed a ruptured dissection of the PA and aorta.
- The scan also showed hemopericardium, hemomediastinum, and blood extravasation into the lungs.
Findings:
- The patient experienced immediate cardiogenic shock post-CT and did not survive resuscitation.
- Concomitant rupture of the pulmonary artery and aorta due to dissection is extremely rare.
- The pathogenesis of this combined dissection remains under investigation.
Implications:
- Multidetector computed tomography (MDCT) is a powerful diagnostic tool for identifying PA and aortic dissection.
- Early diagnosis via MDCT could enable timely surgical repair in survivors.
- This case underscores the critical, albeit rare, association between PA dissection and aortic dissection.
