Summary

Cytoreductive surgery combined with intraoperative intraperitoneal chemotherapy offers a promising therapeutic option for patients with peritoneal carcinomatosis. This approach achieved a 100% one-year survival rate with minimal complications in our initial experience.

Purpose of the Study:

  • To assess the efficacy of cytoreductive surgery and hyperthermic intra- and early post-operative chemotherapy for peritoneal carcinomatosis.
  • To evaluate patient selection criteria for this aggressive treatment modality.
  • To analyze peri-operative complications and patient outcomes.

Main Methods:

  • Retrospective review of nine patients undergoing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (April 2000-October 2004).
  • Inclusion criteria: no extra-peritoneal/hepatic spread, patient fitness, and gastrointestinal primary tumors (colonic, appendiceal, gastric).
  • Data collected on operative details, intensive care unit stay, feeding, hospitalization, complications, and survival.

Main Results:

  • Median age 55 years; 7 women, 2 men. Median operation time: 12 hours 55 minutes.
  • Seven patients required chest tubes; median SICU stay 1 day, median hospitalization 16 days.
  • No peri-operative mortality; one major complication (11.1%). Median follow-up 16 months; median disease-free survival 8 months. Four patients had no recurrence; 100% one-year survival.

Conclusions:

  • Peritonectomy and intra-operative intraperitoneal chemotherapy are viable options for select patients with diffuse peritoneal metastases.
  • Early chest tube insertion was crucial to manage chemotherapy leakage into the thoracic cavity.
  • Appropriate patient selection can lead to good disease-free and overall survival with manageable morbidity.
