    • A 65-year-old male presented with a massive infected congenital central liver cyst (3400 ml pus).
    • Escherichia coli was identified as the causative agent.
    • The patient also had multiple other congenital liver cysts and a renal cyst.

    Findings:

    • Successful treatment involved pus evacuation, partial cyst excision, and drainage of the cyst and subphrenic space.
    • Non-infected cysts were managed with a deroofing technique.
    • The patient remained symptom-free one year post-surgery.

    Implications:

    • This case underscores the importance of considering infection in congenital liver cysts, even if rare.
    • Aggressive surgical management, including drainage and cyst wall management, can lead to favorable outcomes.
    • Early diagnosis and appropriate intervention are crucial for managing complicated congenital liver cysts.
