Purpose of the Study:

  • To evaluate the interobserver reliability of MCA pulsatility index (PI) measurements.
  • To compare PI reliability at proximal versus distal sampling sites of the MCA.
  • To determine the clinical applicability of MCA PI measurements based on observer agreement.

Main Methods:

  • 100 singleton pregnancies (24-40 weeks) with normal fetal growth were included.
  • Two independent operators measured MCA PI at proximal and distal sites.
  • Intraclass correlation coefficients (ICCs) and Bland-Altman analysis assessed interobserver agreement.

Main Results:

  • Peak systolic, end-diastolic, and time-averaged maximum velocities were higher proximally than distally.
  • MCA PI was significantly higher at the distal compared to the proximal site.
  • ICCs for PI were low (0.3 and 0.33), indicating moderate interobserver reliability. Approximately 30% of PI differences exceeded 0.5.

Conclusions:

  • Moderate interobserver reliability in MCA PI measurements limits agreement between observers.
  • The observed variability may hinder the clinical utility of MCA PI calculations.
  • Further standardization of measurement techniques is needed to improve reliability.
