Background:

  • A 76-year-old male presented with a soft tissue mass near the left elbow's biceps brachi tendon.
  • The patient had a history of pulmonary tuberculosis since age 30.
  • Initial diagnosis was bicipitoradial bursitis based on clinical presentation and imaging.

Observation:

  • Magnetic resonance (MR) imaging revealed a mass with specific signal intensities on T1- and T2-weighted sequences.
  • Needle biopsy was performed, followed by surgical excision of the mass.
  • Histopathological examination identified an epithelioid cell granuloma with central necrosis.

Findings:

  • The excised mass was confirmed to be tuberculous in origin.
  • This represents the first reported case of tuberculous bicipitoradial bursitis.
